This course develops advanced competencies for providing care to individuals with complex medical conditions and their caregivers, focusing on assessments that involve family and community, and interprofessional strategies to address social determinant of health. Students will gain skills in service integration and learn about chronic health conditions, with emphasis on cultural and identity-related disparities. The curriculum includes critique of behavior change theories and practical skills for effective team collaboration and consumer engagement. Upon completion, students will be able to offer holistic, culturally sensitive care to enhance the health and well-being of this population.
